The Cardiac Prevention and Rehabilitation Clinic (CPRC) is an outpatient program designed to help you reduce your risk factors and prevent progression of the heart disease. The program includes counselling, education and exercises.
Who do we serve?
The CPRC is for adults 18 years of age and older and their families who are living with heart disease or are at risk of developing cardiac disease.
How do we help our patients?
We offer individual consultations, educational group sessions and exercise programs. Our goal is to help you return to your everyday life while achieving and maintaining a healthy lifestyle. A friend or a loved one may accompany you at the education sessions.
How to access our services?
Your family doctor or your healthcare provider must refer you to the clinic.
